2021 Hands-on Patch Test 2-Day Training

I consent to receive communications from the Contact Dermatitis Institute/Hands-on Patch Test Training by phone, e-mail, fax, and regular mail as it pertains to information about the training. I agree that all notices, disclosures, and other communications provided by the Contact Dermatitis Institute/Hands-on Patch Test Training satisfy any legal requirement that such communications be in writing. The Contact Dermatitis Institute/Hands-on Patch Test Training will not sell my name or contact information to other third parties.

I consent to the Contact Dermatitis Institute/Hands-on Patch Test Training using any photos, other images taken of me, and any results of conference related activities that occurred during the conference for media outreach efforts, promotional campaigns, various publications/communications, or other business purposes as determined by the Contact Dermatitis Institute/Hands-on Patch Test Training.

In the event that the Contact Dermatitis Institute/Hands-on Patch Test Training should find it necessary to cancel or postpone this conference for any reason, including but not limited to low registration, strikes, acts of nature, war, terrorists acts, or other circumstances beyond the control of the Contact Dermatitis Institute/Hands-on Patch Test Training, then the liability of the Contact Dermatitis Institute/Hands-on Patch Test Training shall be limited to the refund of registration fees paid to the Contact Dermatitis Institute/Hands-on Patch Test Training for this course.
